Roadmap for Chronic Disease Prevention (PDF)

Local health departments across the country are leading efforts to reduce adult smoking, increase physical activity, and promote activities that improve health and reduce costs associated with chronic disease. Building on the Operational Definition of a Functional Local Health Department, NACCHO identified the key functions that local health departments should perform to implement chronic disease programs effectively. This roadmap provides guidance on building the capacity needed to implement population-based strategies to prevent chronic disease regardless of an agency's staffing level, legal authority, or population size.
