Mobilizing for Action through Planning and Partnerships (MAPP) 2.0 is NACCHO’s flagship framework for community health improvement. It is a community driven strategic planning framework to improve public health and achieve health equity. MAPP 2.0 was launched in July 2023 and is the update to the original MAPP framework (2001). MAPP 2.0 includes a three-phase process to conduct a comprehensive community health assessment and develop an actionable community health improvement plan. This material includes: a handbook of guidance for the three-phase process, a Starting Point Assessment tool, three assessment tools (Community Partner Assessment, Community Status Assessment, and Community Context Assessment), and a supplemental Power Primer.
