In 2009, Denver Public Health won a NACCHO Model Practices Award for the program “Utilization and Implementation of Point-of-Care Coronary Heart Disease (CHD) Risk Score in a Large Public Healthcare Setting.” This case study summarizes the winning program and outlines how other local health departments can implement similar coronary heart disease prevention programs.
