In 2007, the Jefferson County (CO) Department of Health and Environment won a NACCHO Model Practices Award for the program “La Buena VIDA (Victory in Diabetes Awareness): A Promotor Approach to Diabetes Education and Lifestyle Modification.” This case study summarizes the winning program and outlines how other local health departments can implement similar “promotora” outreach programs.
